  • With this 4 digit pin you will also need a photo id, photo of yourself (passport type photo is ideal) and state registration fee (if applicable) or low income documents to request a waiver for the fee.
  • You will then create an account with the Virtual Gateway and finish setting up a patient account.
  • Bring your temporary paper Program ID Card and photo id to the dispensary, you will receive your plastic card in 1-3 weeks.
